New Patient Registration

register_now_pink_signWhen registering with Drumragh Family Practice you will be registering with the Practice and not an individual GP. For administrative purposes your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by- case basis. This preference will be recorded in your computer record and we will do our best to respect your choice. However, not all the doctors in the practice provide every service and specific doctors may not be available immediately.

If you wish to register to contact the practice for more information

Persons applying for National Health Service registration for the first time please use the HSCR1 form and provide the supporting documentation as requested on the registration form and a recent utility bill and photographic I.D. Please see attached HSCR1

Alternatively if you are currently registered within the NHS or have been within the last 12 months please use the HS200 form. The practice also requires a recent utility bill and photographic I.D. Please see attached HS200

All persons wishing to register are also asked to complete a new patient health summary. Please see attached Health Summary and for children under 6 please complete attached UNDER 6 Registration Questionnaire also, children under the age of 6 years will not be accepted without a copy of Childhood Immunisations/Vaccinations.

Your medical card/ registration form will be forwarded to Business Services Organisation who will issue you with a new Medical Card. This takes approximately 6 weeks.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice. Patients registered with a local practice will be required to make an appointment with the Doctor who will then decide whether the practice will accept you. If the Practice does not wish to accept you and you have difficulty in getting registered you should contact:
